The African American Health Coalition (AfAHC) addresses Social Determinants of Health (SDOH)—like food access, economic stability, and healthcare—to uplift Houston’s Black communities. Through nutrition programs, chronic disease prevention, and improving access to care, we aim to extend life expectancy by 5 years by 2035, fostering equity and healthier futures.
