Link Health

Link Health is a 501(c)3 digital equity program that currently partners with health centers and mobile clinics in Houston that predominantly serve Medicaid populations to increase Affordable Connectivity Program enrollment. These community health centers hold at least one in-person event per month so that every week they are able to execute two events across the Link Health network using the organization’s materials, approach, and Link Health digital dashboards, which track the numbers of in-person sign-ups achieved in their events. We also engage in novel in-person opportunities with an aim to partner with local health fairs, vaccination clinics, and major private partners like Walgreens (Health Corner) and CVS (MinuteClinic) to hold in-person events in their locations.

Additionally, we focus on equipping the mobile clinic and healthcare staff with the necessary knowledge and tools to conduct basic ACP eligibility screenings. Our remote team at Link Health creates a streamlined training program to ensure that these staff have a clear understanding of the process. Once a potential patient is identified, our dedicated remote navigator team will take over, completing and submitting the ACP application on the patient's behalf. This strategic collaboration and partnership, particularly with mobile clinics and CHCs will enable us to extend the benefits of the ACP to a larger demographic, ensuring improved healthcare accessibility for those who need it most.
